Coventry Health Care HMO Members
Coventry Health Care HMO members (previously PersonalCare HMO) who utilize a primary care physician (PCP) at Christie Clinic recently received a letter from Coventry Health Care indicating that Christie Clinic will not be a part of the Coventry HMO network after December 31, 2011. If you received this letter, you have been identified as someone who will no longer be able to utilize their PCP after January 1, 2012, and therefore, have a qualifying event which allows you to change to a new health plan.
Members wishing to elect to a new health plan must contact the State Universities Retirement System (SURS) for a change form. The form, along with a copy of the letter from Coventry Health Care, must be returned to SURS prior to March 1, 2012.
Members who return the form prior to January 1, 2012, will have coverage under the new health plan effective January 1st. Members returning the form after January 1st, but before March 1st, will have coverage effective the date the request was signed and returned to SURS.
This qualifying event does not apply to Coventry Health Care Open Access Plan (OAP) members (previously PersonalCare OAP) since members enrolled in the Coventry OAP do not have a PCP and can access any provider for services.
